To claim a prize by mail, players must send the following in a single envelop to the address listed below:

  • A photocopy of the front and back of the winning ticket, ensuring the barcode is visible, the information form on the back of the ticket is completed, and the ticket is signed*
  • A photocopy of the front and back of your photo ID
  • A completed lottery claim form
